Reading Response Transportation Policy
GUIDELINES AND PROCEDURE


Participants must complete an application.

Participants may qualify for free or sliding scale cost, depending on your household income level.

Participants must be a Reading resident

Transportation is provided for medical appointments both in and outside of Reading.

If a person is requesting additional transportation assistance the request must be approved by the Town Treasurer.

After qualifying for the program, participants will need to call Reading Elder/Human Services at 781-942-6608 at least 24 hours in advance to schedule an ride.

The participants should only call the transportation company when the are ready to be pick-up from a medical appointment.

Each participant completes an annual re-certification of need to remain in this program.
