Healthcare and Home Care
Our Social Worker is available to assist you with the following healthcare issues:
Advanced Directives - Healthcare Proxies - DNR Orders - Health Insurance Applications (Medicare, MassHealth, etc) - Rehabilitation and/or Nursing Home Placements - Coordination of Services - Hospitalization Discharge and/or Aftercare Planning.  Call Marie Ammer, LICSW at 781-942-6659.

Our Nurse Advocate can help seniors understand their medical problems, medications and treatments.  She will help them develop strategies for living safely and assist them in planning for their future needs. The Nurse Advocate is also available to advocate and help seniors navigate the complex healthcare system. If you or someone you know could benefit from this type of assistance, call 781-608-5271.

Reading Response Program - In-Home Health Care
This program is designed to meet the home health needs of Reading resident who meet income eligibility guidelines . Through this program residents can receive nursing care, home health aide services and/or the Lifeline Emergency Response System. The program is offered and funded by the Town of Reading’s Hospital Trust Funds and is administered by Hallmark Health VNA. For further information about this program, call 781-338-7800.

SHINE
SHINE stands for Serving Health Information Needs of Elders. This MVES program helps elders and Medicare beneficiaries to understand their Medicare benefits and other health insurance options. The SHINE Program ensures that Medicare beneficiaries have access to accurate, up-to-date information about their health care options. A SHINE volunteer is available at the Reading Senior Center for private consultations on the 4th Friday of every month. Appointments can be scheduled by calling 781-942-6608.
